COVID-19 Vaccination

The Logan County Health District (LCHD) now has Pfizer COVID-19 Vaccine available during walk-in hours.  Walk-in hours for COVID-19 vaccinations are Tuesdays from 9:00 AM – 12:00 Noon and 1:00 – 3:30 PM at the LCHD office.  Participants can also schedule an appointment with LCHD at

Those age 12-17 must have a parent or guardian present at the time of COVID-19 vaccination and are only eligible to receive the Pfizer vaccine.  Those age 18+ are eligible to receive either Pfizer vaccines.  Parents and participants are recommended to bring their Driver’s License or Photo ID and insurance card (if available). 

Individuals who are homebound may call (937) 651-6244 to arrange in-home vaccinations. 

Logan County Health District is now using the state’s online scheduling system.

Please schedule your appointment based on the schedule below:

Monday – Appointment Only – Moderna Only (1st, 2nd, and Additional Doses) – 9:00 AM – 12:00 PM

Tuesday – Walk-In Hours – All Vaccines – 9:00 AM – 12:00 PM & 1:00 PM – 3:30 PM

Thursday – Appointment Only – Pfizer Only (1st, 2nd, Additional and Booster Doses) – 9:00 AM – 12:00 PM

Friday – Appointment Only – J&J (Janssen) Only (Single Dose *NO BOOSTER*) – 1:00 PM – 3:30 PM

If you do not have internet access, your options are:

1.  All six locations of the Logan County Libraries have computers with internet access for the public to use.  

2.  Call LCHD at 937-592-9040 option 6 to leave a message for a volunteer to call you back and assist.  This option may take longer to get registered as we may not be able to return your call immediately.
